Requests for Information

District 128 encourages requests for information to be submitted via Personal Delivery, U.S. Mail or Fax to any one of the appointed FOIA Officers at the Community High School District 128 Administration Center, 50 Lakeview Parkway, Suite 101, Vernon Hills, IL 60061, or 847-247-4543 (FAX)

All requests for access to information under the Illinois Freedom of Information Act should be directed to one of the district's Freedom of Information Officers:
